Entry Forms for the 2016 Quilt Show

Entry Form Registration opens April 1

Each show entry must be registered separately

Regisgtration deadline is JULY 5th

On-Line Registration Please use this method if at all possible. This is the most efficient - saves your paper and postage AND data entry time for the Registrar. After registering you should receive a receipt from the Registrar within five days.
In Person The registrars will provide registration help at various announced venues throughout the county.

Insurance Waiver

You will need only one copy of the signed Insurance Waiver.

Word Template Open this file on-line. If you have MS Word, you can save the file to your computer for your records and fill in the information for each entry using your computer before printing.
PDF Use this choice if you have Adobe Reader and plan to complete the form manually.

Paper Labels

A paper label must be tacked to the back of every item entered in the show. Tack only at the corners. Do not sew down all edges.

Boutique Consignment

You will need a signed Insurance Waiver with your Boutique Inventory form. The waiver is the second page of either document.

Instructions Describes rules and policies of the Boutique as well as an explanation of the forms and procedures used.
